Over the past fifteen years, most surgical specialities have developed less invasive techniques (navigation, endoscopy, robotic surgery, etc.). Spinal surgery has benefited from these innovations through the application of microscopic surgery (or microsurgery), video-assisted, endoscopic and navigated surgery. Open surgery techniques continue to be used without compromising the effectiveness of neurological release or mechanical stabilisation, resulting in less osteomuscular damage. The scar is smaller and the deep structures are less exposed. The mechanical concept is based on atraumatic muscle dilatation using dedicated retractors.
Vertebral stabilisation by pedicle face and filling can be performed using the same percutaneous principle.
INDICATIONS: remain the same as for open surgery, but the consequences of retraction and dislocation are significantly reduced, thus reducing operative bleeding, postoperative pain, the length of hospital stay and a quicker return to activity.
In collaboration with Radiology, infiltrations of the entire spine are carried out under SCANNER before proposing surgery, depending on the indications.
In collaboration with the Anaesthesia Department, lumbar spine operations can be carried out under locoregional anaesthesia, epidural, with an anaesthetist specialising in pain management. This practice avoids the consequences of general anaesthesia, is better tolerated in general and in particular in elderly patients than GA, and postoperative analgesia is prolonged. Patients stand up 4 hours after the end of the operation.
