Access to your medical file

You can request a copy of your medical file or of certain documents kept in it. If you wish, a doctor can be present to show you the documents.

You have just lost a loved one

Here is some information to help you with your application:

• You should go to the funeral home of your choice, which will help you organise the funeral.

Please note:

A list of authorised funeral directors in departments 93 and 75 is available from the clinic reception desk (tel: 01 48 97 72 00). We would be grateful if you could inform the care service of the time at which the body will be collected.

• The death certificate is issued by the doctor responsible for your loved one. It will be given to the funeral directors you have chosen, so that they can carry out the legal formalities. If the funeral home does not provide this service, the certificate will be given to you so that you can carry out the formalities yourself at Bagnolet town hall (Place Salvador Allende 93170 Bagnolet - 01 49 93 60 00).

If you wish to see your loved one, please contact the head of the department where they were hospitalised.

• You can pay your respects in the room we provide.
• Preservation care and dressing are carried out exclusively by funeral directors.
• The deceased's personal effects will be handed over to you by the ward's nursing staff.
